POSITION SUMMARY: 

The R.N. Supervisor oversees and supervises the overall nursing service/operation of the Skilled Nursing Facility while promoting and restoring the health of the residents through day-to-day management and long-term planning. The R.N. Supervisor directs and develops Skilled Nursing staff; collaborates with physicians and multidisciplinary professional staff, and provides physical and psychological support to residents to ensure quality care is being provided to the residents daily. Along with the DON, the RN Supervisor is accountable for nursing compliance, excellence, and delivery of resident care services in adherence with Company, local, state and federal regulations.

ESSENTIAL FUNCTIONS:

  1. Assists in developing, reviewing, revising and updating resident Plans of Care for individual residents. Assists in completion of MDS form.
  2. Works with various departments to coordinate resident admissions, transfers and discharges.
  3. Participants in resident assessments and ensures that notification is given to the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within our established policies.
  4. Performs assessments on residents for end of life and pronounce / document date and time of death.
  5. Makes resident rounds to ensure appropriate care is being rendered, identifying and making corrections as necessary. Rounds with attending physician and Wound Nurse as necessary.
  6. Responsible for medication administration and management. Reviews medication orders and medication sheets for completeness and accuracy of information, accuracy in the transcription of physician orders, and adherence to stop order policies.Works on a medication cart / treatment cart if directed to support the nursing team.
  7. Performs IV management including; starting IV’s, administers IV antibiotics, add meds to IV bags, and adds ABT/vitamins/minerals as ordered.
  8. Assist in completing restraint assessments, pain assessments, oral assessments, psychotropic assessments, bowel and bladder assessments and I & O weekly evaluations.
  9. Maintains accurate and timely reporting and recording of information regarding condition of residents in accordance with departmental protocols.
  10. Completes injury/incident reports, medical forms, reports, evaluations, studies, charting, etc., for residents and staff as necessary.
  11. Makes written and oral reports/recommendations to the DON concerning operation of nursing staff and nursing services to ensure consistent delivery of care and promotion of resident dignity and rights.
  12. Meet with nursing personnel to assist in identifying and correcting problem areas and/or the improvement of resident care, infection control, and other universal precautions.
  13. Maintains documentation of resident care services by auditing resident and department records to ensure that all nurse’s notes are charted in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care.
  14. Assists in orientation and training of newly assigned personnel in job duties on unit as well as supervise and instruct other nursing personnel to ensure compliance with facility policies and procedures. Assists in the job performance evaluation of nursing staff. Assists in the coaching of staff in areas of job performance, conduct, and quality of care delivered.
  15. Assists the Charge Nurses and other Nursing personnel in performing nursing procedures as necessary.
  16. Provides administrative and clinical support to SNF in the absence of the DON as needed.

NON ESSENTIAL DUTIES:

  1. Participates in facility surveys (inspections) made by authorized government agencies as requested.
  2. Participates in the facility’s QAPI Program as needed and assigned.
  3. Performs and assists with other related duties and special projects as needed and assigned.
  4. Attends and participates in department meetings and other required trainings.
  5. Adheres to all Company and departmental policies and procedures.
  6. Provides leadership by working cooperatively with nursing assistants and other Nursing co-workers.
  7. Provides encouragement, guidance and resources to staff and residents when needed.
  8. Encourages positive interaction between staff and residents.
  9. Improves professional knowledge through professional reading, and in-service education.
  10. Promotes a positive calm environment on assigned group and respond to emergencies in a calm, confident and professional manner.
  11. Performs all duties in a safe manner in accordance with facility safety standards, ensuring staff follow infection control procedures and universal precautions at all times in accordance with facility procedures for the safety, health and welfare of staff, residents, and guests.
  12. Maintains the highest level of confidentiality at all times.
  13. Operates computer and programs necessary to perform job including other business office machines.
  14. Maintains business/professional boundaries with all staff, management and outside vendors.

MINIMUM QUALIFICATIONS:

  1. Graduation from an accredited School of Nursing.
  2. Current California –R.N. (Registered Nurse) license.
  3. Minimum one year previous supervisory experience.
  4. Minimum one year experience and/or training in long term nursing care/ skilled nursing preferred.
  5. Valid CPR and First Aide or BLS certification.
  6. Knowledge regarding the nursing process and its application, including the input of assessments, planning, implementation, state and federal regulations and evaluation of nursing care.
  7. Knowledge of basic infection control procedures.
  8. Ability to demonstrate good theory base as well as good clinical skills and participates in delivering direct patient care as needed.
  9. Ability to demonstrate leadership, communication and organizational skills required.
  10. Ability to make independent decisions when circumstances warrant such action.
  11. Effective interpersonal skills and the ability to work with an interdisciplinary care team.
  12. Able to communicate and relate positively, favorably and cooperatively with others, including employees, residents, family members, and personnel of outside agencies and organizations.
  13. Ability to supervise, organize, implement, and interpret programs, goals, objectives, policies and procedures that are necessary for providing quality care while maintaining compliance with state and federal regulations.
  14. Performs well with minimal supervision and organizes work load so that assigned duties and patient care can be completed within the guidelines and according to facility policies and procedures as well as state and federal regulations.
  15. Basic computer knowledge and skills. Experience in Point Click Care software preferred.
  16. Ability to read, write and communicate to residents, staff, families, vendors, and all business relationships in English language.
  17. Ability to pass a criminal background clearance check, physical, and TB test.
  18. Ability to function as a member of the team in a multi-task environment.
  19. Flexible and available to be contacted in order to provide a professional service if necessary, but not formally on duty.
